SERVICE Directory

OUTPATIENT

The treatment of psychiatric disorders through scheduled therapeutic visits between the therapist and the individual. Group and/or family therapy may also be available.

 

MEDICATION MANAGEMENT

A qualified prescriber will evaluate an individual’s need for psychotropic medications, prescribe, and provide ongoing medical monitoring of those medications.

ASSERTIVE COMMUNITY TREATMENT

The Assertive Community Treatment (ACT) provides high-intensity services in the community to include treatment, rehabilitation, and support activities using a comprehensive treatment team model for individuals with severe and persistent mental illness.

EMERGENCY COMMUNITY SUPPORT

Home and/or community-based case management program for individuals in a behavioral health crisis. ECS provides referrals and connections to community providers and assistance with daily living skills.

DAY REHABILITATION

Provides learning activities, daily structure, and support for successful living in the community for individuals with severe and persistent mental illness.

DAY TREATMENT

This service provides psychotherapeutic and psychoeducational programming in a structured environment.

SUPPORTED EMPLOYMENT

Provides recovery and rehabilitative services and supports to individuals as they work to obtain and maintain employment.

MENTAL HEALTH RESPITE

A safe, protected, supervised residential environment on a short-term basis, which provides support, linkages, and assists in the transition back into the community.

HOSPITAL DIVERSION-CRISIS INTERVENTION

This service is a hospital diversion program assisting individuals 19 and older who are experiencing a crisis in their lives. The service is operated by peer specialists and offers a 24 hour warm line along with a safe, comfortable, and calm setting to visit in times of crisis.

COORDINATED SPECIALTY CARE

Comprehensive and coordinated behavioral health services including psychotherapy, medication management, family support, and education, care coordination and supported employment and education services are provided by a team of professionals for those ages 14-35 who have recently begun to experience psychosis.

PROFESSIONAL PARTNERS

The Professional Partner Program provides voluntary, family-centered case management services based on the wraparound model for severely emotionally disturbed children, young adults, and their families. The program is designed to help families achieve stability, improve functioning, and community integration.

YOUNG PEOPLE'S PEER GROUP

A consumer-focused service for young adults 16-26 that supports dignity, self-advocacy, and empowerment and provides some assistance in crisis/relapse prevention through the encouragement of continuing recovery goals and action planning.

INDEPENDENT LIVING CLASSES

Independent Living Classes are offered on a variety of life skills topics designed to provide young adults with the knowledge and skills necessary to live an independent lifestyle.

ACUTE INPATIENT

Inpatient hospitalization provides psychiatric treatment for individuals meeting medical necessity criteria who have mental health and/or co-occurring disorder and are experiencing an acute psychiatric condition.

 

PSYCHIATRIC RESIDENTIAL REHABILITATION

A residential program that provides individualized treatment and recovery inclusive of psychiatric rehabilitation and support for individuals with a severe and persistent mental illness and/or co-occurring disorder.

SECURE RESIDENTIAL

A residential treatment program that provides individualized recovery, psychiatric rehabilitation, and support for individuals with severe and persistent mental illness and/or co-occurring substance use disorder demonstrating a moderate to high risk for harm to self and others.

COMMUNITY SUPPORT

Provides direct rehabilitative and support services to individuals focusing on daily living skills, etc. in the consumer’s home.

PEER SUPPORT

Peer support uses an empathetic approach to empowering individuals with mental health and/or substance use disorders through supporting, motivating, and advocating for those we work with to move toward recovery. Peer support is provided by trained individuals in our community who have lived experience with mental health and/or substance use disorders and have successfully moved into a recovery lifestyle.

CRISIS RESPONSE

Crisis Response uses natural supports and resources to resolve an immediate mental health or substance use crisis in the least restrictive environment by creating a plan with the individual to resolve the crisis. Crisis response is typically activated with law enforcement to engage with the consumer on scene with the goals of implementing a crisis intervention plan, ensuring safety, accessing the necessary level of care, and resolving the crisis in the community if possible.

Referrals to the mobile crisis team from the community must go through the Nebraska Family Helpline or 988.

DUAL DISORDER RESIDENTIAL

A highly structured residential treatment program for individuals with a primary substance use disorder and co-occurring severe and persistent mental illness. The program provides integrated treatment and works to stabilize mental health systems while engaging the individual in a program of rehabilitation and recovery.

ASSESSMENT MH

A Mental Health Assessment is a comprehensive biopsychosocial, strengths-based assessment that involves the process of gathering information to assess functioning, determine if the symptoms meet the diagnostic criteria for a mental health or co-occurring disorder, and identify treatment needs.

INTENSIVE COMMUNITY SERVICE

Designed for individuals with serious mental illness, including those with co-occurring disorders, who experience frequent and debilitating symptoms resulting in high rates of use of acute or other intensive levels of care.

 

 

RECOVERY SUPPORT

Recovery Support links individuals to community resources, identifies and problem solves barriers that limit independent living, and builds on strengths and interests that support well-being.  Includes crisis relapse prevention, active case management and referral to other services.

SUB-ACUTE INPATIENT

Provides stabilization, support, engage the individual in comprehensive treatment, rehabilitation and recovery activities and transitions the individual to the least restrictive safe setting as rapidly as possible. Subacute inpatient is designed to help resolve the presence of acute or crisis mental health symptoms.

 

EMERGENCY PSYCHIATRIC OBSERVATION

Provides care for typically up to 24 hours in a secure, medically supervised hospital setting for evaluation and stabilization of acute psychiatric symptoms. The service will prevent further exacerbation or deterioration when possible and facilitates transition to the necessary level of care.

WARM HAND-OFF

A Warm Handoff is a therapeutic session that is conducted between two members of the health care team with the consumer and his/her family, in a health care setting, a primary care provider links or connects the consumer to a therapist with the goal of engaging the consumer, identifying immediate coping strategies, developing a plan to address the presenting issue, and using motivational enhancement strategies to engage the consumer towards change.

An evaluation designed to establish the presence or absence of a diagnosable substance use disorder, an assessment of the risk of dangerousness to self and/or others, and recommendations for the appropriate service level with referrals to appropriate service providers.

Provides medical and social services to severe opioid use disorder individuals along with outpatient substance use disorder treatment.

Emergency intervention program available 24 hours a day for individuals experiencing acute intoxication and/or withdrawal.

HALFWAY HOUSE

A transitional supportive living and treatment program located in the community for individuals with substance use disorders. Individuals are able to develop and practice their recovery skills while reintegrating back into the community and working to obtain employment or enroll in school.

SHORT TERM RESIDENTIAL

A short term, highly structured residential treatment program for individuals with a primary substance use disorder that provides therapy and psycho-educational programming.

THERAPEUTIC COMMUNITY

A longer term, structured residential treatment program with peer-oriented, recovery-centered activities that provides therapy and psycho-educational programming.

TRANSITION AGE YOUTH PROFESSIONAL PARTNER PROGRAM

The program is designed to help families achieve stability, improve functioning, and community integration.

PATH

Projects in Assistance to Transition from Homelessness (PATH) includes a medication management clinic and homeless outreach services.

SOAR

SSI/SSDI Outreach Access and Recovery (SOAR) provides assistance in applying for SSI and SSDI benefits.

Services provide coordination, planning, and implementation of substance abuse prevention, and mental health promotion and suicide prevention activities.

A consumer specialist will serve as an advocate and liaison to consumers of behavioral health services within the region.
