Yamhill Access and Community Engagement VISTA
 
 
 
The position joins Oregon Health Authority’s (OHA) statewide AmeriCorps VISTA team. In addition to activities at your individual site, each member receives additional orientation, training throughout the year, team meetings, and support from team leaders and project director. Yamhill Community Care (YCCO) is one of 16 coordinated care organizations (CCOs) in Oregon that manage care and provide resources for members on the Oregon Health Plan (OHP), the state's Medicaid system, serving those experiencing poverty. OHP is a unique Medicaid model which assigns members a CCO, providing a designated primary care provider, services to manage health, and other benefits designed to address the whole person. YCCO has a community-based governing committee, the Community Advisory Council, composed of YCCO members and their family. YCCO is also advised by other local stakeholder committees to inform its decisions. YCCO has an Early Learning Hub focusing on strengthening families, coordinating and aligning services, and improving school readiness. The VISTA member will work with YCCO, its various committees, community-based organizations and county services to help coordinate the best care possible, as well as create and monitor programs that address poverty. YCCO offices are in McMinnville, Oregon. McMinnville is a town of around 33,000 nestled in the heart of wine country, and home to Linfield College. McMinnville is an hour from Portland, an hour from the coast, three hours from Mount Hood, and surrounded by gorgeous hiking and sightseeing opportunities. The OHA AmeriCorps VISTA Partnership Project is committed to building a diverse workforce to strengthen our work in Oregon communities. Member Duties : The AmeriCorps member will improve local capacity to meet community needs, especially for those experiencing barriers because of their language or cultural background: Community Engagement 1. Develop and evaluate multilingual engagement activities and materials Community Improvement Plan 1. Convene community groups to lead locally led health improvement 2. Address equity components of plan Health Related Social Needs 1. Coordinate training on health related social needs 2. Conduct analysis to discover gaps in service based on demographics 3. Promote social needs screening strategies AmeriCorps VISTA members serve through indirect service and capacity building, meaning the work you will do is the behind-the-scenes work. While programs developed endure for years, you will have limited interaction with the community and will serve remote or in an office. Bilingual or bicultural applicants are preferred, and YCCO encourages applicants of all backgrounds and identities.
