What if my spouse is not available to complete the online form?
Medical history questions must be completed by the person (applicant)
requesting coverage (with the exception of a dependent child applicant (see
below)). So a spouse applicant must answer the questions and agree to all
terms prior to submission. If this is not feasible, it may be more
convenient for you to print a paper form and complete the submission by
mail.
May I complete the submission for my child?
Yes. The member may complete the medical history questions and agree to
all terms on behalf of a dependent child applicant.
Is there a time limit for my submission?
Yes. For your protection, the application session will time out after 30
minutes of inactivity. Please be aware that you will not
have an opportunity to save a draft during this time. You must be prepared
to reference the required information listed before you begin your
submission.
I don't know my coverage amounts. Where can I get this
information?
You can get plan-specific information, including your current coverage
and limits for additional coverage, from your benefits
administrator.
I can't remember the specific dates or the physician information. How
should I answer those medical questions?
Please provide as much detail as possible. We will contact you if we need
more information.
When will I know if I'm approved or declined for group insurance
coverage?
Once you submit your evidence of insurability you will be immediately
informed if you are approved for coverage or if your application requires
additional review. If your application requires additional review, you will
be advised of next steps in the process by mail.
Do I need my physician to complete this evidence of insurability
submission?
No. All information must be completed by you, the applicant (or by the
member for a dependent child applicant). If we need additional information
from your physicians, we will contact them directly. Please be sure to list
your physicians' names, locations and phone numbers where
requested.
Will I be asked to have any medical tests?
Requests for additional medical information, including an examination,
blood test and/or urinalysis, are made only in some instances and would be
requested of you by a medical underwriter at The Standard. If required, The
Standard will notify you of the details and provide further
instructions.

Yes. The information you submit is protected by encryption technology to
ensure your confidentiality.
