REACH stands for Racial & Ethnic Approaches to Community Health. REACH is a national program administered by the Centers for Disease Control and Prevention (CDC) to reduce racial and ethnic health disparities.
The program aims to connect Medicare and Medicaid beneficiaries with community services that address health-related social needs, including housing instability, food insecurity, utility needs, interpersonal violence, and transportation.
As a Navigator Program, PA-NH works closely with Access Health CT to raise awareness of health insurance options available to Connecticut residents and offer health insurance application assistance.

Project Access-New Haven (PA-NH) is a 501(c)(3) nonprofit organization dedicated to increasing access to medical care and services for underserved individuals in Greater New Haven. We achieve this mission by mobilizing a volunteer provider network to expand care for uninsured patients and providing patient navigator (PN) and community health worker (CHW) services to help underserved patients access timely medical care and connect with local resources to address health-related social needs.
We have presented our work at the national conferences of the American Public Health Association, AcademyHealth, Communities Joined in Action, Society for General Internal Medicine, Society for Academic Emergency Medicine, and Association of Oncology Nurse Navigators.
Our results have been published in peer-reviewed journals, including Health Services Research, Connecticut Medicine, Medical Care, Journal of Emergency Medicine, Academic Emergency Medicine, and the Journal of Healthcare for the Poor and Underserved.
Many of our staff are Access Health CT Certified Application Counselors.
PA-NH and Yale New Haven Hospital were awarded the 2014 Connecticut Hospital Association and Connecticut Department of Public Health’s Hospital Community Service Award for working together to care for medically underserved Greater New Haven residents.
The PA-NH Breast Health Navigation Program was awarded the 2016 Susan G. Komen of Southern New England Power of Pink Innovative Program of the Year Award.
PA-NH was awarded with the New Haven Medical Association Community Service Award in 2019
