Company Description

The OnTrackNY Program is an innovative treatment program that serves adolescents and young adults who have experienced an initial episode of psychosis. The Primary Clinician will work as part of a small multidisciplinary team providing coordinated and assertive treatment in the earliest stages of illness, as well as outreach and recruitment coordination.

All applicants must comply with CASES/NYC's vaccination policy. Individuals selected for employment cannot begin working until they provide verification that they are fully vaccinated against COVID-19 (with an FDA-authorized vaccine) or submit and gain approval for a Medical or Religious Exemption. Individuals approved for exemption must submit weekly PCR test results.

Travel or Special Conditions/Requirements: Some traveling required

We believe that in-person operations are the best way to serve our clients, both directly and through essential operational support. This role is hybrid with in-person work primarily in Harlem.

Job Description

  • Serves as the point person for the participant and their family;
  • Conduct needs assessments;
  • Engages in safety planning and wellness management planning with participants;
  • Connects participants and families with the services and supports they need;
  • Provide behavioral interventions and take an especially active role in the family support domain.
  • Conducts clinical assessment to determine the participant’s level of social functioning and substance use via a shared decision making (SDM) framework
  • Provides Social Skills Training, Substance Abuse Treatment, Behavioral Activation, Coping Skills Training, and Psychoeducation.
  • Provides psychoeducation regarding first episode psychosis (FEP) and the goals of the OnTrackNY program;
  • Provides supportive therapy;
  • Develops and modifies treatment using a shared decision-making process with clients

Outreach and Recruitment Coordination Duties:

  • Recruiting patients for program through community education and presentations at potential referral sources
  • Serving as primary contact person for referral sources
  • Establishing and maintaining database of referral sources with up-to-date contact information
  • Conducting telephone and in-person eligibility screening of patients referred to program
  • Referring patients ineligible for program to other appropriate community providers
  • Attending weekly team meetings
  • Participating in ongoing training and supervision
  • Rotating on-call responsibilities with other team members

Qualifications

  • Licensed Masters or Ph.D. in an appropriate field (social work, psychology, mental health counseling)
  • Training and experience working with adolescents and/or young adults with serious mental illness.
  • Experience in community-based mental health services

Additional Information

Salary: 70-80K
