Why is patient not part of associationStatus?

      Proposed motion to include subject in the Device.associationState.subject per FHIR-36214, while leaving practitioner out as a choice since there is no clear use case (certainly not 80%+) where a device is associated with a practitioner, only patients.  Note the operationalState.operator does included a practitioner to use the device on/with a patient.

      Also, this would result in removing Device.subject as that is superceded by Device.associationState.subject.
