Note 1: BEFORE starting this form, please ensure that you have a copy of: 1) HUD-1 Final Closing Statement from your original purchase and 2) A copy of your most recent mortgage statement for the first mortgage and any subordinate liens/lines/mortgages. (please do not email attachments)
Note 2: If this is a request to sell or refinance, all timelines are determined by the signed and recorded restriction agreement that was signed during the purchase of the home. No official timeline can begin until the Owner completes the proper Notice of Intent to Sell or Transfer as outlined in the restriction agreement. Each agreement states how each timeline works and how each maximum price is calculated. Calculations and timelines vary from agreement to agreement.
