Building on the Montgomery County Department of Health and Human Services. Public Health Services and Public Health Chronic Disease Working Group adds a new addition that fights against Diabetes. The Unite 2 Integrate Diabetes Coalitions (U2IDC) initial priority is to provide a forum to advance the priorities of the Type 2 Diabetes (T2D) Action Plan in Montgomery County. The U2IDC focuses on assessing needs, raising awareness of the burden of chronic diseases, and promoting uptake of prevention, education, and promotion efforts.

Our goal is to support Montgomery County's high-risk minority residents with managing and controlling T2D through early detection, addressing the social determinants of health, and promoting optimal self-care and rehabilitation.

Our objectives are to cultivate and build a network of stakeholders to work collaboratively to leverage T2D information and Montgomery County resources. As well as provide a platform for coalition members to share their latest diabetes research findings and evidence of best practices, and build a comprehensive, accessible and reliable source of diabetes prevention and management resources to share amongst coalition members and community.
