Updated cell phone activation instructions for enrollees of Foundational Community Supports

The directions for activating cell phones for Foundational Community Support enrollees has changed. You can no longer activate phones by following “start here” instructions included in the manufacturer box.

Please follow the new directions below to activate all cell phones.

New directions to activate cell phones:

  1. Go to the activate site to start activation.
  2. Enter in the IMEI or SIM number of the device and select submit.
  3. On the next page, enter the zip code for where the device will be activated and select submit.
  4. You will receive a success notification on your device.
  5. Put the SIM card back inside the device and power it on.
  6. You will receive a text message confirming activation and providing you with a newly assigned phone number.
