Description This form may be utilized to document fuel transactions that are exempt under WAC 173-441 and WAC 173-446. Fuel users may complete and present this certificate to fuel suppliers, including suppliers that are covered entities, to document the exempted fuel usage for record keeping and reporting purposes.
