Youth ID Application

The Parks and Recreation Department Aquatic Facility Rules require anyone age 7 and older to provide proof of identification and East Hartford residency for entry into all town aquatic facilities. 

Acceptable forms of identification/residency for youths ages 7-15 are:

  • Current East Hartford Public Schools ID, OR
  • Parks and Recreation Youth ID

Parks and Recreation Youth IDs are issued by the Parks and Recreation Department for children ages 7-15. The fee is $6.00 for an ID card which is valid for 3 years. We do not provide ID cards for residents ages 16 and up.  For in person photos, our office is now open to the public.

In order to obtain a Parks and Recreation Youth ID, the following must be submitted:

  • A copy/photo of the parent/guardian’s photo ID with East Hartford address, AND
  • A high-quality photo image of the child, AND
  • Copy of Child’s Certificate of Birth, OR
  • East Hartford Public Schools report card

REQUIRED DOCUMENTATION TO PROVE IDENTIFICATION/ RESIDENCY

Photos must be high quality/in focus and child should be facing the camera

Waiver of Liabiltiy
I submit that the information stated above is truthful and accurate, and that the child for whom I am requesting a youth ID is a permanent resident at the address listed above. I understand that the East Hartford Parks and Recreation Department may contact the school listed to confirm the home address and that the child is currently enrolled. The East Hartford Parks and Recreation Department also reserves the right to request additional information confirming residency (mortgage/lease/rental agreement/utility bills) if necessary. I understand that East Hartford Aquatic Facility Rules require proof of residency for anyone entering the facilities, and I understand that children 11 and under must be accompanied by a responsible adult age 16 or older, at all times while in the facility. I agree to abide by all facility rules, and understand that pool use is a privilege that may be revoked at any time.
